Under the New Zealand Public Health and Disability Act 2000 (NZPHD Act) all DHB's are required to produce three accountability documents to advise the Minister of Health, Parliament and the communities they serve of their intentions, planned activities and expected outcomes.
Together with national policy, these documents help to maintain transparency and enable robust review of the intentions of both the Ministry and of DHB's. The documents provide a long term planning element to demonstrate capacity, sustainability of services and best use of resources. The use of short, medium and long term performance targets in these documents also assists in evaluating DHB progress and effectiveness.
The Canterbury DHB's Planning & Funding Division is responsible for producing these key accountability documents on behalf of Canterbury DHB.

| District Strategic Plan (DSP) | The District Strategic Plan
describes the DHBs directions and long term goals over a 5-10
year period and the strategies to achieve those goals. The
DSP is aligned to the Government's health and disability
strategies, is developed based on the DHB's HNA and is
influenced by the revenue available to the DHB and feedback from
the community as part of a formal public consultation process.
The DSP must be approved by the Minister of Health and is

| Statement of Intent (SOI) | The Statement of Intent (SOI)
provides a high level focus on key financial and non-financial
objectives and targets. The SOI has a three year focus and
is the formal accountability document between the DHB and
Parliament. It describes what the DHB intends to achieve
over the next three years in the terms of reducing in inequities
and improving the health and well being of the community.
the SOI includes a set of medium-term outcome measures to
demonstrate progress towards the longer-term objectives set in

| Annual Report | The DHB's performance against the

| District Annual Plan (DAP) |
The District Annual Plan details how the DHB will achieve the medium and long-term goals and objectives set in the SOI and DSP. The DAP has a one year focus and outlines activity planned for the coming year that will assist the DHB not only to achieve its goals but also to meet its immediate challenges and fulfil the expectations and requirements of the Minister of Health. The DHB also presents specified outputs in terms of targets against Ministry-set measures or ‘Indicators of DHB Performance’ which are then reported to the Ministry as part of the DHB’s Crown Funding Agreement reporting. | Crown Funding Agreement (CFA)

The Crown Funding Agreement is the contract between the DHB and the Minister of Health which links the DAP to the funding provided by the Ministry of Health and the performance required from the DHB. |
The Canterbury DHB reviews its long-term direction every three years, as part of the development or review of its DSP. The DSP is aligned to the Government’s health and disability strategies and is developed based on the DHB’s HNA, and influenced by the revenue available to the DHB and feedback from the community as part of a formal public consultation process.
The DHB produced its most recent HNA in 2004, followed by a public consultation process to establish its direction for the five years 2005-2010. The DHB’s DSP ‘A Healthier Canterbury - Directions 2010’ was produced and signed off in 2006. In recognition of the specific local needs of Canterbury residents, the DHB established five Core Directions, five Strategic Health Gain Priorities and four Disease Priorities as part of the DSP process.

These Core Directions and Strategic Priorities influence the development of the DHB’s SOI and DAP and determine much of the activity that is undertaken in any given year and where additional focus will be placed to better meet the needs of the Canterbury population. Around these Priorities the DHB has developed a number of local strategies and action plans to focus activity and influence improved outcomes for the Canterbury community.
