How to enrol with a dentist

During Year 8 of school:

During Year 8 you will receive a purple transfer form from the School Dental Service so you can be transferred to the Free Dental Service for Teens. Fill in your details and choose a dentist. Click here to find  a dentist. Return the form to your school dental clinic, or if it is not possible, return it to your chosen dentist. If you have any questions call 0800-TALK TEETH (0800 825 583).


You are entitled to a free check-up every 12 months, so make sure you take advantage of it!

After Year 8 of school, but before turning 18:

If you are not enrolled with a dentist, follow these simple steps to enrol:

  1. You can try your local dentist, or click here to find a dentist on our website. 
  2. Ring, or drop by the dentist, ask if they can enrol you for the Free Dental Service for Teens and arrange an appointment.
  3. If you have any questions call 0800-TALK TEETH (0800 825 583).
